While we were saddened to learn of the closure of the Fayette C.A.R.E. Clinic, we want you to know that the Healing Bridge Clinic (HBC) is here to continue the quality care you have been receiving. The HBC has long shared a mission with Fayette CARE to provide compassionate, high-quality healthcare to those of you in our community who need it most. We are honored to welcome you into our clinic family and ensure that your medical needs are met without interruption.

Review our eligibility requirements: Because we are a non-profit clinic serving uninsured and underserved individuals, we invite you to view our current eligibility requirements below.

Request an Appointment: Once you have reviewed the requirements, you can conveniently request an appointment directly through our online portal.

Medical Records: We recommend requesting a copy of your medical records from Fayette CARE Clinic as soon as possible to assist our providers in understanding your health history. To request records, contact Fayette CARE at 770-719-4620 or visit the clinic at 105C Bradford Square, Fayetteville, GA to complete a records release form.

WHAT TO BRING TO YOUR FIRST APPOINTMENT

  • List of current medications
  • List of current medical conditions
  • Valid and current photo I.D. with current address
  • Proof of residency such as a utility bill from the previous month with your name and address
  • If you live with someone else, a Notarized Affidavit is required from the homeowner
  • Proof of household income from the past 30 consecutive days (paystub, social security statement, note from employer on company letterhead verifying pay for cash jobs)
  • “No Wage Letter” for yourself (and your spouse, if applicable) from the Georgie Department of Labor if not receiving wages
  • If supported by another person, you must bring a letter stating such, signed by that person, along with a copy of their photo ID
